What to expect at your first visit:

On your first visit to our offices, we make every effort to relieve any discomfort you have. We make sure your first visit with the doctor is thorough and will include:

  • Medical history review

  • Time to ask questions

  • A consultation explaining diagnosis and treatment option

  • Same day treatment if possible

What to bring to your first visit:

Please arrive early to complete the registration forms or you can download them below and bring them completed. You must bring:

  • Drivers license or picture ID

  • Insurance Card

  • If you have any medical conditions, please bring along a list of your medications

 

We need to be aware of your medications and long term medical status, in case your healing ability is affected.

 

Being prepared for your appointment will help ensure that we have all of the necessary information to provide you with the best possible care.
